Formasty MCP MCP server

Formasty provides an official MCP endpoint that allows AI agents to programmatically create, read, update, and publish forms and quizzes. It enables agents to build lead qualification forms, applications, and recommendation quizzes with scoring and conditional workflows, which can then be reviewed and published by the user.

OAuth sign-in: the first time you connect, your client opens the vendor's own authorization prompt in a browser.
